File Upload not working?

Hi all,

 

I have created a file upload script but it keeps failing for some reason?

What exactly happens when you try to upload the file? Do you get the message you specified in the else section? Have you tried setting the permission on the upload folders to a permission level which does allow for uploading and modifying files?
