Could I just put the Terms & Conditions of a project in the invoice and would it count as a legal agreement? Stuff like "2 revisions, 1 contact form, logo design, 1 year hosting" etc.

I guess you could, but you might be better off adding it to a contract and calling it a contract. Then the client can't say things like I didn't know it was final etc. The word contract also makes it seem more legally binding.
