Hi Guys,

 

I am in the middle of a big development for a new business a friend is launching.

 

He is wanting me to create a registration page were a user enters the relevant details that the form states.

 

Then all the details are to be emails to his email address.

 

I have basic knowledge of PHP form and have designed the Contact page which was a basic setup.

 

However this page requires text fields, tick boxes and file uploads.

 

I have attached a image of what he would like it to look like below:

 

 

 

There is to be a rule in there that makes sure that the user fills in each data field, ticks at least one box from each questions and uploads the 2 relevant files, then agrees to the T&C's.

 

Can anyone assist or point me in the direction of something that will help me.
