  1. Sketch on paper is always the best way to getting start it. But my suggestion to you is to find some sample template online, and pick the one which fits the best what you are looking for and show it to your web designer, therefore, he/she could easily meet what you are really looking for on a website you want to make
  2. For me mostly on paper, but right now I just saved all kinds of template, and just use it for layout which realli save me a lot of time and it is easy to change the content later on as well
  3. It really depends on what kind of website you are making. I always use flash to do all kinds of show room website for example, a restaurant website, and any image related website. For others, I would just use html first and add some flash later on to make the site more interesting.
