Questionnaire under User Name & Password Control

I need to find a simple solution to a problem I have. I need to add to a leukaemia support website a questionnaire comprising of about 50 form fields, tick boxes, drop downs, free text, etc. The unique feature is that it has to be under User Name and Password control such that entered data can be retrieved and modified at a latter data.

 

I have looked at a few options, Content Management Systems, but these are just too complicated and over the top for what I need.

 

Could anyone suggest a source of free code that Password Control that I could easily just add/modify the questionnaire details to make a working system?

Take a look at Google Forms - very easy to use and has all the features you need.

 

Might not be the best option if you need to embed it into your website. You could always place the link to the form and brand it to match your website?

Thank you for the link to Google Forms, nice approach. However, I really need to keep patients on the my website as they are entering sensitive information.

 

Would there be any other proposals?
