Default Value Text Box = URL address

Hi all,

I have a custom 404 page and would like a form on the page in which the user can submit to show the error.

On this form a want a 'read only' text box that automatically copies the URL into the text box so therefore the user doesn't have to enter this and I can see what address they are trying to reach.
