I'm quite new to CSS so I was wondering how I can create this widget for "my account"? I have attached a jpg.

Hi There,

 

For the rounded input fields take a looky at this:

For the rounded and non-rounded corners on the border of the box, you could either use images, or try some css fluid borders like this:

http://www.webreference.com/programming/css_borders/

Depends if you have a background image or plain colour in behind the box.
