Need help from Magento Experts

I having problem with my Table Rate Shipping method which is whenever user(malaysia region) is at Shipping Method step, it will display

Sorry, no quotes are available for this order at this time.

I think I have configured it correctly, my config is as follows:

I'm try to solve this out these few days but no luck, hope that someone can guide me.

Yes I already done that, Default Country, Allow Countries and Postal Code is Optional for the following countries were set to Malaysia.

does it make any difference if you use region 494?

I did a little modification at my shipping_tablerate(sql) as follows and it works:




but whenever customer from region_id 494 and dest_zip 94600 bought items, my magento ignore custom shipping cost I setup for those region and zip

those region and zip shipping cost should be 10.000 but it displayed 0.0000

Any Idea?
